Core Responsibilities

Care Captains provide stability and accountability across the Core C.A.R.E. network. They act as the primary point of contact for assigned families, ensuring each household’s needs are met and progress is tracked through our support programs.

  • Maintain regular monthly check-ins with assigned families.
  • Track and update needs, milestones, and progress in HubSpot.
  • Coordinate with Care Coordinators and Fulfillment Specialists to ensure timely support.
  • Encourage families to engage with available Core programs and community events.
  • Report urgent needs or issues to the Executive Director for rapid response.
  • Attend volunteer updates and occasional leadership meetings.
